FULL TIME NURSE/EDUCATOR

We are seeking registered nurses to join our Health Care Quality Unit (HCQU) in our office in Hershey. The HCQU is designed to assure that individuals who have developmental disabilities (DD) are as physically and behaviorally healthy as they can be.

Educational/Professional Requirements:

  • R.N. with an unrestricted Pennsylvania License.
  • Minimum of 3 years experience.
  • Behavioral health experience preferred.
  • Experience with individuals who have DD preferred.

General Description:

  • Monday through Friday.
  • No Holidays or Weekends.
  • Single destination travel.
  • Perform assessments of the physical and behavioral health of individuals who have DD.
  • Develop and/or deliver education programs.
  • Collaborate with DD service providers and other physical and behavioral health care providers.

FULL-TIME Service Coordinator

We are seeking a Service Coordinator (SC) (Scranton Office) to provide assistance to individuals who have physical disabilities or older adults who need assistance with obtaining daily living services.

Responsibilities:

  • Conduct outreach calls to consumers;
  • Complete an initial visit with consumers to develop a service plan;
  • Complete an assessment summary, obtain information concerning services, establish goals, develop an emergency back-up plan, schedule future face-to-face visits and communicate provider choice and consumer rights with each consumer;
  • Enter the service plan into the HCSIS or SAMS system;
  • Coordinate with service providers to establish services for the consumer;
  • Complete a Service Authorization Form with the provider;
  • Monitor and conduct face-to face or telephone visits as required;
  • Rotate and share “on call” responsibilities with other SC staff; and
  • Other duties as assigned.

Essential Skills and Experience:

  • Planning: ability to think ahead; and
  • Communication: good communication skills – written and verbal.

Preferred skills and experience:

  • Bachelor’s degree in related field or related experience; and
  • Knowledge of human service agencies;
